Stokvels
Fill in a stokvel membership form online
Upload the club's membership form and DocFilla fills the club's details. Send each member their own copy; they fill in their ID number, address and next of kin on their phones and it comes back to you.
A stokvel's member register is one completed form per member: who they are, how to reach them, who their beneficiary is and when they joined. Chairs and treasurers collect these forms on paper, by photo on WhatsApp, or not at all. DocFilla sends each member their own copy to complete on a phone, and every completed form comes back to one place.

Upload your stokvel membership formHow it works for this form
- 1
Upload the membership form, or make one from the bank's template. Fill in the club's name, the contribution and the joining date rules, and keep it as a template.
- 2
Send each member their own copy by email. Every field marked for them is required, so a form cannot come back half done.
- 3
Members fill in their names, ID number, address, phone, next of kin and beneficiary, and sign. Each completed form returns to the chair's or treasurer's Documents.
What the form asks for
- The stokvel's name and the member's membership number
- Member's full names, ID number, date of birth and contact details
- Physical address
- Next of kin or beneficiary, with their ID number and relationship
- Date joined, contribution amount and payment method
- For a burial stokvel: the dependants covered
- Declaration that the member accepts the constitution, with signature and date
What to attach
- A copy of the member's ID
- Proof of address, where the constitution requires it
- Beneficiary's ID, for a burial stokvel
Mistakes people make
- No beneficiary named, so a payout on death has nobody to go to.
- A phone number that changes without the register being updated.
- Members who never signed the declaration accepting the constitution.
- Keeping the register only on one person's phone.
Questions about this form
- Do members need email to receive the form?
- To receive their own copy to complete, yes. A shared signing link, which needs no email, collects signatures but not a whole form.
- Where do the completed forms go?
- To the Documents of whoever sent them, each as its own document, with a PDF to download for the club's records.
- Is members' information safe?
- What members fill in is encrypted under the sender's key before it is stored, is never sold or used to train AI, and is deleted on the schedule the sender sets.
