Fill in a burial society claim form online

Upload the society's or insurer's claim form and DocFilla fills the claimant's names, ID number, address and bank details. You add the deceased's details, sign, and attach the death certificate and IDs.

A burial society or funeral policy claim is made at the worst time, and the form is the same every time: who died, who is claiming, the policy or membership, and where to pay. DocFilla fills the claimant's details from saved details so only the deceased's particulars are typed, and a chair can send the form to the bereaved member to complete on a phone.

How it works for this form

  1. 1

Upload the society's or insurer's claim form. Both fillable and printed versions work.

  2. 2

The claimant's names, ID number, address, contact details and bank details for the payout fill from My details. The deceased's names, ID number, date and cause of death, and the policy or membership number are typed in.

  3. 3

Sign the declaration, download, and submit the PDF with the supporting documents the society or insurer lists.

What the form asks for

  • Policy or membership number, and the society's or insurer's name
  • Deceased's full names, ID number, date of birth, date and place of death and the cause
  • Relationship of the claimant to the deceased
  • Claimant's full names, ID number, address and contact details
  • Bank details for the payout, in the claimant's name
  • Funeral parlour details, where the society pays the parlour directly
  • Declaration, signature and date

What to attach

  • A certified copy of the death certificate
  • The notification of death, the DHA-1663, which the doctor or undertaker completes
  • Certified copies of the deceased's and the claimant's IDs, and the beneficiary's if different
  • Proof of the claimant's bank account
  • A police report for an unnatural death
  • A birth certificate for a child

Mistakes people make

  • Claiming into a bank account that is not in the claimant's name.
  • Uncertified copies of IDs; most societies and insurers want certified copies.
  • Leaving the cause of death blank, which sends the form back.
  • Missing the DHA-1663 for a death that has not yet been registered.

Questions about this form

What documents does a funeral claim usually need?
A certified death certificate, the notification of death (DHA-1663, the form the doctor or undertaker completes), certified IDs of the deceased and the claimant, and proof of the claimant's bank account. Unnatural deaths need a police report. Check the society's or insurer's own list, because they differ.
Can the chair send the claim form to the bereaved member?
Yes. The chair sends the form with DocFilla; the member completes and signs it on a phone and the copy returns to the chair to submit with the documents.
How quickly is a funeral claim paid?
That depends on the society or insurer and on the documents being complete. A form with every line filled and certified copies attached is the fastest way through.
