Fill in a leave application form online

Upload your employer's leave form and DocFilla fills your name, employee number and department. You pick the type of leave, the dates and sign; your manager signs after you.

Leave forms are the same form every time: who you are, which leave, which days, and two signatures. DocFilla keeps your part filled in, so you type the dates and nothing else, and your manager can sign the same form in turn. Teams keep the form as a template and send it whenever someone asks.

How it works for this form

  1. 1

Upload your employer's leave form, or ask HR to send it to you from their DocFilla template.

  2. 2

Your names, employee number, department and manager fill from My details. Tick the type of leave and type the first and last day, and the number of working days.

  3. 3

Sign on your phone. The form goes to your manager to sign in turn, and the completed copy returns to HR.

What the form asks for

  • Employee's full names, employee number, department and position
  • The type of leave: annual, sick, family responsibility, maternity or parental, unpaid or study
  • First and last day of leave, and the number of working days
  • Leave balance, on some forms
  • Contact details while away, and who is standing in
  • Employee's signature and date, and the manager's approval signature

What to attach

  • A medical certificate for sick leave of more than two consecutive days, or where the employer asks for one
  • Proof of the event for family responsibility leave, where the employer asks
  • A study timetable for study leave

Mistakes people make

  • Counting public holidays and weekends as leave days.
  • Applying for sick leave as annual leave because the certificate is still coming.
  • Submitting the form after the leave instead of before, where the policy requires approval first.
  • Leaving the stand-in line blank.

Questions about this form

How much leave am I entitled to?
The Basic Conditions of Employment Act sets the minimum: 21 consecutive days of annual leave a year for a five-day week, paid sick leave equal to six weeks' working days over a 36-month cycle, and three days of family responsibility leave a year after four months' employment. Your contract or a bargaining council agreement may give more; check your own.
When do I need a medical certificate?
An employer may ask for one when you are off sick for more than two consecutive days, or more than twice in eight weeks, and need not pay sick leave without it.
Can my manager sign the same form after me?
Yes. Send the signature fields as a signing round: you first, then your manager. Each sees the form as it stands when their turn comes.
