Fill in a debit order mandate online

Upload the debit order mandate and DocFilla fills the account holder's names, bank, branch and account number, one digit per box. You add the amount and the date and sign.

Landlords, schools, insurers and gyms collect by debit order, and every one of them has a mandate form asking for the same banking details in slightly different boxes. DocFilla fills the account holder and the bank details from your saved details, one digit per printed square, and leaves the amount, the collection date and the signature to you. Many collections are now confirmed through your bank as a DebiCheck mandate as well.

How it works for this form

  1. 1

Upload the mandate form as a PDF or a photo. Character boxes for the account number and branch code are filled one digit per box.

  2. 2

Account holder's names, ID number, bank name, branch code, account number and account type fill from My details. The amount, the first collection date and the reference are typed in.

  3. 3

Sign on your phone and download. If the collector uses DebiCheck, your bank will ask you to approve the same details on its app or by USSD.

What the form asks for

  • Account holder's full names, ID number and contact details
  • Bank, branch code, account number and account type
  • The amount, or the maximum amount, and how often it is collected
  • The first collection date and the day of the month
  • The collector's name, the agreement or policy reference, and the abbreviated name that appears on your statement
  • The account holder's signature and the date

What to attach

  • Proof of the bank account: a stamped letter or a recent statement
  • A copy of the account holder's ID
  • The lease, policy or contract the collection is for

Mistakes people make

  • Transposing digits in the account number. Check it against the bank letter before you sign.
  • Using the universal branch code when the form asks for the branch's own code, or the other way round.
  • Signing for an open-ended amount when the agreement names a fixed one.
  • Approving a DebiCheck request from your bank without reading the amount and date it shows.

Questions about this form

What is DebiCheck?
A debit order mandate that you confirm electronically with your own bank, on its app, online banking or by USSD, when the agreement starts. The bank keeps the amount and date you approved, and disputes are limited to those terms. A paper mandate is still signed, but the DebiCheck approval is what the bank checks.
Can I cancel a debit order?
You can stop it through your bank, but the agreement you signed may still be owed. Cancel the agreement with the collector in writing as well.
Are my banking details safe on DocFilla?
Saved banking details are encrypted before they are stored, under a key that never leaves the server, and are used only to fill your own forms.
